Donation Request Form

Chicago Winery donates gift cards for raffles, wine packs for auctions, and does a very limited number of wine sponsorships. If you are interested in being considered for a donation please fill out the form below. We will contact you if we are able to donate to your cause. If you have not heard from us within 30 days, we are unable to fulfill your request. Please feel free to submit another request for a future event.
